Office Hours:
The order window hours are:
8:00 a.m. - 12:45 p.m. and 2:00 p.m. - 4:30 p.m., Monday/Wednesday/Friday
Due to staff shortages, until further notice, Vital Statistics will be closed on Tuesdays and Thursdays.
The window is closed from 12:45 p.m. - 2:00 p.m. daily.

The Vital Statistics Office serves as the registrar for all births and deaths occurring in Washoe County from the late 1800’s to current. Birth and death certificates are confidential in the State of Nevada and may only be released to a qualified applicant. A qualified applicant is defined as the registrant, or a direct family member by blood or marriage, his or her guardian, or his or her legal representative. For more information, refer to NRS 440.650 and NAC 440.070.

Birth & Death Certificates

If you are a qualified applicant, you can complete an application form and mail it with a check or money order, payable to Washoe County District Health Department. Certified copies of birth records are $13.00 per copy and certified copies of death records are $11.00 per copy.

Valid photo identification is needed to obtain a certificate.

Certificates can be ordered by one of the following methods:

1) Mail a check or money order made payable to Washoe County District Health Department, along with a completed Birth Certificate Application* (PDF, 1.01 MB) or Death Certificate Application* (PDF, 1.15 MB) and a copy of your photo identification to:

* Note - Estimate 3-4 weeks for processing and receipt of your request. Requests for which no record is found are subject to the established $8.00 search fee.

2) Appear in person at the office which is located in the Washoe County District Health Department, 1001 E. Ninth Street, Reno, Building B, first floor, Birth and Death Records. The order window hours are: 8:00 a.m. - 12:45 p.m. and 2:00 p.m. - 4:30 p.m., Monday/Wednesday/Friday. Due to staff shortages, until further notice, Vital Statistics will be closed on Tuesdays and Thursdays. The window is closed from 12:45 p.m. - 2:00 p.m. daily.
