Immunization Fees

What You Need

Bring all immunization records for the individual(s) receiving immunizations. Our immunization registry may not contain immunizations given at other clinics, private physicians, or the school district. It does not contain records of immunizations given out of state.

We strongly encourage parents or court-appointed legal guardians to personally accompany their child for immunizations. There are forms, questionnaires, and consents that need to be read, completed, and signed by the parent or legal guardian.

We do not accept notes from parents authorizing a third party such as a step-parent, aunt, uncle, or adult sibling to bring a child in for immunizations. If a child comes with a relative or friend, paperwork will be provided for them to take home for the parent to read, complete, and sign before the child can be immunized.

How to Access Immunization Records:

HIPAA regulations prohibit us from faxing records to schools and/or parents or even discussing an individual's immunization records over the phone.

Washoe County residents need to pick up immunization records in person on Monday, Wednesday or Friday between 8:00 a.m. to noon or 1:00 to 4:30 p.m. (closed noon to 1:00 p.m.)

Immunization records will not be released to a third party such as a step-parent, aunt, uncle, adult sibling or to a parent of a child/children 18 years and older with out a completed Authorization for Release of Information form (PDF, 47.5 KB). It you are a legal guardian, you need to bring appropriate documentation.

Individuals living outside of Washoe County need to complete an Authorization for Release of Information form and fax it to 775.328.6102. Please complete all areas highlighted in yellow. Please allow 3-5 working days for processing.
